Physical Therapy New Patient Information
At the time of registration at ProHealth Physical Therapy and Pilates Studio, you must provide the following:
- Photo ID (with coordinating statement)
- Insurance card
ProHealth PT and Pilates is a participating provider for most insurance plans including Medicare, Blue Cross Blue Shield, Aetna, United Healthcare, Cigna, and Humana.
